Medical Records Information

The medical records are in reality medical case histories which include complete details of a diseased person and the details of his/her ailments. A medical record is particularly required when the person intends to visit a new physician or submit health related details at his place of work either as a supportive evidence for his long term absence or for claiming specified medical benefits. It is always wise to have a detailed medical record along with you for the benefit of your own proceedings.

The Significant Contents of a Medical Record

• Your personal identity that is your name, your date of birth, your blood group and your contact details
• The date and time when you had your last physiological check up
• What have been the results of several screenings and tests and when were they conducted
• What are the kinds of illnesses you are suffering from and when and what are the types of surgeries you have gone through.
• What medicines are you having recently, their dosages and their tenure of usage?
• Are you allergic to any particular medicinal group?
• Are you suffering from any sort of long standing disease?

The Formatting of a Medical Recording

Usually a medical record is written on a paper or documented in folders. The folders are systematically divided into multiple sections. Each section will tell you about a new kind of disease experienced by the affected individual. You should always differentiate between the past medicals records and those which are recent.  What you have already suffered and what you are going through now should not be mixed up.

Electronic Medical Recording

Electronic medical recording is the norm now. This has created a sort of revolution in the essential formatting of several medical recordings. This is indeed a very simplified process which saves your time and at the same time enables you to arrange your medical data appropriately.

The Role of HIPAA Forms in Matters of Medical Recording

It was in the year 2003 that the ”Privacy Rule of the Health Insurance Portability and Accountability Act” came into being on the 4th of April. According to the privacy rule of this medical act a special dimension has been given to the way “protected heath information” is utilized and made known to the covered and concerned individuals.

HIPAA falls under federal legislation and is legally applicable to New York’s worker’s procedures of compensation. Thus, in this case the New York’s Worker’s Compensation Board has nothing to do in the proceedings and implementation of this particular legislation.

Laws Regarding Medical Recording

There are laws regarding both the protection of medical details for social benefit of an afflicted individual and laws for where the person has to go through several medical check ups in order to get employment in a governmental agency.

If a patient is not satisfied with a particular treatment procedure he can go through his personal EMR or electronic medical record and get a fair idea about the details of the treatment he/she has been receiving. An electronic medical record helps to maintain a better transparency between the doctor and the patient.
